Abstract

Chronic diseases such as diabetes, hypertension, cardiovascular diseases, and chronic respiratory disorders are major global health concerns. Effective management of these conditions requires a multidisciplinary approach, where clinical pharmacists play a crucial role in optimizing patient outcomes. This paper explores the expanding role of clinical pharmacists in chronic disease management, emphasizing their contributions to medication therapy management, patient education, adherence monitoring, and interprofessional collaboration. By providing personalized pharmacotherapy, ensuring medication safety, and offering lifestyle counseling, clinical pharmacists help in improving treatment efficacy and reducing hospitalizations. The study highlights the impact of clinical pharmacists in enhancing healthcare quality and patient-centered care for chronic disease management.
